P0401 — Exhaust Gas Recirculation Flow Insufficient
P0401 means the EGR system is commanded to flow and is not. It is almost always carbon blocking the passages rather than a failed part, and it will fail an emissions test.
The engine runs normally in most conditions. You may notice pinging under load or a rough idle, and it will not pass an emissions test. This code will cause an emissions test failure.
What P0401 means
Exhaust gas recirculation lowers combustion temperature by returning a measured amount of inert exhaust to the intake, which cuts nitrogen oxide formation. The module commands the valve open and expects a response — a change in manifold pressure, or a reading from a dedicated position or temperature sensor. When the expected change does not appear, it sets P0401. Because the gas being recirculated is full of soot, the passages carbon up over years, and cleaning them resolves this code far more often than replacing the valve does.

The valve itself seizes, usually from the same carbon. It can often be moved by hand with the engine off, which distinguishes a stuck valve from a blocked passage.
Control solenoid or vacuum supply failed
On vacuum-operated systems a split hose or a failed solenoid means the valve is never actually commanded, however healthy it is. Cheap to check and easy to overlook.
Flow or position sensor reading wrongly
The module infers flow from a sensor. A sensor reading incorrectly reports insufficient flow while the valve is opening normally — the fault is the measurement, not the flow.
Common misdiagnosis
Confirm the valve is actually not moving before replacing it. A sensor fault produces this code with a perfectly functional EGR system.
How to diagnose it
Command the valve open and watch idle quality
With the engine idling and warm, command the EGR valve open. A working system will make the idle stumble noticeably as inert gas displaces the charge.
Remove the valve and inspect it
Take the valve off and look at the seat and the pintle. Try to move it by hand. Heavy carbon on the seat, or a pintle that will not move, is the answer.
Safety
The valve and its housing stay hot long after the engine is switched off.
Inspect and clean the EGR passages
With the valve removed, check the ports in the manifold. On a high-mileage engine they are often reduced to a fraction of their original diameter.
Check the control signal and any vacuum supply
Confirm the solenoid receives its command and, on vacuum systems, that vacuum actually reaches the valve. A split hose produces this code with an otherwise perfect system.
